 note = {本研究の目的は、ギアチェンジ（治療目的を治癒以外に方向転換し症状緩和を主とした治療の選択に変更していく時期）を告げられ意思決定する前のがん患者と、意思決定したがん患者の役割にはどのような変化が起きているのかを明らかにすることである。入院中の患者はどのような役割があり、他者からどのような役割を期待されているのか、「看護師から患者への役割期待」、「医師から患者への役割期待」、「家族から患者への役割期待」「病院から患者への役割期待」の4 つに分類して文献検討を行った。その結果、ギアチェンジ期におけるがん患者の役割期待は、ギアチェンジポイントを境に看護師・家族・医師の役割期待は変化すると捉え、ギアチェンジ期にあるがん患者の役割期待と病院からの役割期待を概念枠組みの図にまとめた。ギアチェンジ前は看護師、医師、家族はがん患者に対して、「積極的に病気と闘ってくださいという期待」があり、治療を行うが積極的治療にも限界が生じる。その瞬間が、ギアチェンジポントである。ギアチェンジ後、「症状緩和を最優先にしてくださいという期待」へと変化する。その一方で、病院のルールを守ってくださいという「病院からの患者への役割期待」に変化は起きていないことを導いた。これらのことから、看護師はがん患者の役割を知ることで、そのがん患者に合った接し方や看護の方法を工夫することができると示唆された。, The purpose of this study is to clarify how the role of a cancer patient has been changing before and after the patient makes a decision toward the changing gear. The changing gear which is the period shifting the therapeutic purpose from cancer reduction to palliative care focusing on symptom reduction. In order to identify the patient’s roles and the patient’s roles expected from others during hospitalization, we examined the related documents by classifying the roles into four categories. The categories include “the roles expected from the nurse to the patient,” “the roles expected from the doctor to the patient,” “the roles expected from the family to the patient,” and “the roles expected from the hospital to the patient.” As a result, for the roles of cancer patients in the changing gear period, the expectation from the nurses, the family, and the doctors have changed before and after the changing gear point. In addition, we showed the summary of the expected roles of cancer patients in the changing gear period and the roles expected from the hospital in a figure of conceptual framework. Before the changing gear , the nurses, the doctors, and the family have the “expectation to fight thedisease actively” for the patient. Although the treatment to cure the cancer is performed, the treatment also has the limit. That moment is a changing gear point.After the changing gear, the patient has been shifted to the “expectation to put the first priority on the symptom relief.” On the other hand, we found that “the role expected from the hospital to the patient” that is to follow the hospital rules has not changed through the changing gear. From these findings, it was suggested that the nurses can improve their attitude and the nursing methods corresponding to the individual cancer patient by understanding the roles of cancer patients.},
