 note = {本研究は、看護師と医療ソーシャルワーカー（以下MSW）が「ギアチェンジ」の時期にあるがん患者とその家族へどのような看護と援助を行っているのかを考察したものである。看護師とMSW へのインタビューデータは修正版グラウンデッド・セオリー・アプローチ（M-GTA）の手法で分析した。　先行研究によると「ギアチェンジ」とは、がんの症状が抑えきれなくなり今までの集中的治療を中止して、苦痛な症状の緩和を積極的に行う治療へと移行していく時期を呼ぶ。「ギアチェンジ」の時期は、がん患者や家族にとってこれまでの治療経過を通して築いてきた医療者との関係を断ち切り、新たな環境の中で治療を受けていくことになる。このような時期のがん患者に対し、医療職である看護師と福祉職であるMSW がどのような看護と援助を提供し、連携しているのかを明らかにした。　分析の結果、医師より「ギアチェンジ」について説明をされた後のがん患者と家族の反応は、『がん患者・家族の退院後の方向性が一致しているもの』、『がん患者・家族の転院後の方向性が一致しないもの』『がん患者・家族共にギアチェンジを納得していないもの』の３つに分類された。その内容から、「ギアチェンジ」にあるがん患者の経過とともに看護師とMSW が提供する看護と援助を連続的な視点で明らかにした。, In this study, we investigated the ways of the care and the assistance that are provided by nurses and medical social workers(MSWs) for the cancer patients in the 'gear change' period and their families. The data obtained through the interviewswith those nurses and MSWs are analyzed by the Modified Grounded Theory Approach (M-GTA).According to the precedent study 'gear changing,' is the word that means a transitional period when the intensive medicalcare is changed to the palliative care because of the incurability of the symptomatic state of the patient.In the 'gear change' period, the patient and his or her family would finish the relationships with the medical staffs that have been constructedthrough the therapeutic course and start to receive the palliative care in the new environment. We investigatedhow the medical staffs such as nurses and the welfare staffs, namely MSWs, cooperate and provide the care and the assistanceto the patients in the 'gear change' period.The interview revealed that the response of the patient and the family who received an explanation for 'gear change' bythe doctor are classified into three types, 'the patient and the family share a same objective about the discharge', 'the patientand the family have different objectives about the discharge', and 'both the patient and the family disagree to changethe gear'. Using the data, we evaluated the clinical course of the patient in the 'gear change' period and the care and theassistance that are provided by the nurses and the MSWs from the continuous perspective.},
